Job description:
- Providing care and support as per the Care/Support Plan
- Developing ways to promote the individuals rights and choices
- Providing information, advice and support to families and other professionals
- Update all recording systems using clear and understandable writing or grammar, including finances, A&I, Risk Assessments, Daily Care, Diaries, Care Plans, Support Plans & Logs
- Monitoring behavior patterns and behavior charts
- Personal care
- Ensuring protection against abuse
- Adhering to our & the client’s Policies and Procedures
- Providing vital life skills training individually tailored to increase independence, choice and control
