Job description:

  • Providing care and support as per the Care/Support Plan
  • Developing ways to promote the individuals rights and choices
  • Providing information, advice and support to families and other professionals
  • Update all recording systems using clear and understandable writing or grammar, including finances, A&I, Risk Assessments, Daily Care, Diaries, Care Plans, Support Plans & Logs
  • Monitoring behavior patterns and behavior charts
  • Personal care
  • Ensuring protection against abuse
  • Adhering to our & the client’s Policies and Procedures
  • Providing vital life skills training individually tailored to increase independence, choice and control
